A white or almost white crystalline powder; odourless; hydroscopic; darkens gradually on exposure to light or heat.Action and indications: Vincristine sulfate is an alkaloid extracted from Vinca rosea. As a cell cycle specific drug, it can inhibit growth of neoplasm and formation of proliferating cell spindle, as a result, mitosis is stopped at its metaphase.
1. Mechanism of action
Inhibition of microtubule formation: Vinblastine binds to microtubule proteins, preventing them from polymerizing into microtubules (a key structure of the cytoskeleton), resulting in the inability to form spindles during mitosis, causing the cell cycle to stagnate in the mid stage (M phase), thereby inhibiting cancer cell division.
Selective effect: It has a significant effect on rapidly proliferating tumor cells (such as malignant tumor cells in the blood system), but also has some toxicity to normal cells.
2. Indications (clinical application)
Changchun alkaloids are mainly used to treat the following malignant tumors:
Acute lymphoblastic leukemia (ALL), especially childhood ALL, is often used as a core drug in combination chemotherapy regimens (such as VDLP regimen).
Lymphoma:
Hodgkin's lymphoma (HL)
Non Hodgkin's lymphoma (NHL)
Other tumors:
Multiple myeloma (combined with other drugs)
Rhabdomyosarcoma, neuroblastoma (solid tumors in children)
Some solid tumors (such as small cell lung cancer, breast cancer, but less used).
3. Pharmacological characteristics
Administration route: intravenous injection (not oral or intramuscular).
Metabolism and excretion: mainly metabolized in the liver, excreted through bile, and a small amount excreted through the kidneys.
Neurotoxicity is prominent: it is easy to pass through the blood-brain barrier, but has less toxicity to the central nervous system, mainly causing peripheral neuropathy.
