1. Pharmacological mechanism of action
Antibacterial effect:
By inhibiting the peptidyl transferase activity of the 50S ribosomal subunit in bacteria, blocking peptide chain extension, and thus inhibiting bacterial protein synthesis, it has a broad-spectrum antibacterial effect.
It is effective against both Gram positive bacteria (such as pneumococcus and staphylococci) and Gram negative bacteria (such as typhoid bacilli, influenza bacilli, and Escherichia coli), and also has inhibitory effects on chlamydia and rickettsia.
Other mechanisms:
Inhibit the stability of hypoxia inducible factor-1 alpha (HIF-1 alpha) in hypoxic cells and reduce the mRNA expression levels of ( ) and glucose transporter 1.
2. Indications
Infectious diseases:
Febrile and paratyphoid fever (one of the preferred medications).
Bacterial meningitis (especially infections caused by Haemophilus influenzae and meningococcal bacteria).
Severe respiratory infections (pneumonia, pertussis).
Eye infections (such as conjunctivitis, keratitis) and skin and soft tissue infections.
Special pathogen infection:
Rickettsia disease (such as typhus), anaerobic bacterial infection (adjuvant therapy).
3. Pharmacokinetic characteristics
Absorption and distribution:
Oral absorption is fast, reaching peak blood drug concentration within 2 hours; Widely distributed in tissues and body fluids throughout the body, the concentration in cerebrospinal fluid can reach 30% -50% of blood concentration.
The half-life of plasma is about 2.5 hours, but due to insufficient metabolic capacity in newborns, the half-life is significantly prolonged.
4. Adverse reactions and risks
Hematological toxicity:
Bone marrow suppression (dose-dependent, such as anemia and leukopenia) and aplastic anemia (rare but fatal).
Gray baby syndrome:
Newborns may suffer from cyanosis, circulatory failure, and even death due to insufficient liver metabolic capacity.
Other: Gastrointestinal reactions (nausea, diarrhea), allergic reactions (rash).
5. Drug resistance
Bacteria can produce chloramphenicol acetyltransferase (CAT) inactivating drugs or alter the structure of ribosome targets through genetic mutations.
Drug resistance is more common in Escherichia coli, Shigella, and other bacteria, while the resistance rate of typhoid bacilli is relatively low.
