1. Pharmacological mechanism of action
Inhibition of dihydrofolate reductase (DHFR): By selectively inhibiting the DHFR enzyme of protozoa such as malaria parasites and Toxoplasma gondii, the synthesis of tetrahydrofolate is blocked, interfering with DNA/RNA replication and protein metabolism, leading to pathogen death.
Inhibition of multidrug resistance transporter protein (Mdr-1/P-glycoprotein): reduces intracellular drug efflux, enhances intracellular concentration and efficacy of other antimalarial or chemotherapy drugs.
2. Indications
Malaria:
Used for etiological prevention (such as prevention before entering malaria endemic areas) and anti recurrence treatment (in combination with sulfonamide drugs, such as sulfadoxine).
It has a killing effect on extracellular parasites of Plasmodium vivax, Plasmodium vivax, and Plasmodium falciparum.
Toxoplasmosis: combined with sulfadiazine for the treatment of Toxoplasma encephalitis in immunocompromised patients (such as AIDS).
3. Combination therapy enhances efficacy
Combined with sulfonamide drugs (such as sulfadoxine), a synergistic anti malarial effect is achieved by double blocking the folate synthesis pathway (sulfonamide inhibits dihydrofolate synthase, and ethambutol inhibits DHFR).
4. Other applications
Research field:
Used for studying multidrug resistance mechanisms and developing reversal agents (such as inhibiting Mdr-1/P-glycoprotein).
As a DHFR inhibitor, it is used for research on tumor cell metabolism and drug resistance.
5. Precautions
Toxicity risk: Long term or high-dose use may cause bone marrow suppression (such as megaloblastic anemia) and gastrointestinal reactions (nausea, vomiting).
Contraindications: Prohibited in early pregnancy (teratogenic risk); Patients with severe liver and kidney dysfunction need to adjust their dosage.
