Cefotaxime is indicated for severe infections caused by susceptible bacteria, including bacterial pneumonia, complicated urinary tract infections (e.g., pyelonephritis), severe skin and soft tissue infections, genital infections (e.g., gonorrhoea), intra-abdominal infections (e.g., peritonitis), bacterial meningitis, endocarditis, and borreliosis. It is also used for bacteraemia associated with these infections and for perioperative prophylaxis in high-risk surgeries, such as colorectal procedures, often combined with an anti-anaerobic agent.

Application&Function

1. Uricosurics: Probenecid interferes with the renal tubular transfer of cefotaxime, increasing its exposure approximately 2-fold and reducing renal clearance by half at therapeutic doses. While no dosage adjustment is needed for patients with normal renal function due to cefotaxime's large therapeutic index, adjustments may be necessary for those with renal impairment.  

2.Aminoglycosides and Diuretics: Like other cephalosporins, cefotaxime may enhance the nephrotoxic effects of drugs such as aminoglycosides or potent diuretics (e.g., furosemide). Renal function should be closely monitored in patients receiving these combinations. Additionally, cefotaxime should not be combined with bacteriostatic antibiotics (e.g., tetracyclines, erythromycin, chloramphenicol) due to the potential for antagonistic effects.  

3.Laboratory Test Interference: Cefotaxime, similar to other cephalosporins, may cause a positive Coombs' test, which can interfere with blood cross-matching. It may also yield a false-positive glucose reaction with reducing substances (e.g., Fehling's solution), though this does not occur with glucose oxidase methods.
