Diphenhydramine, an antihistamine. It has the effect of antihistamine H1 receptor, has a strong inhibitory effect on the central nervous system, and atropine-like effect. It is suitable for allergic diseases of skin mucosa, such as urticaria, hay fever, allergic rhinitis, etc. It can also be used to prevent motion sickness such as seasickness, car sickness and plane sickness. Sleepiness and dry mouth were the main adverse reactions.
Application&Function

Pharmacological action
It is a derivative of ethanolamine, the antihistamine effect is less than promethazine, the effect duration is shorter, and the sedation effect of the two drugs is the same. It also has local anesthesia, antiemetic and anti-M-cholinergic effects.
(1) Histamine action: It can compete with histamine released in the tissue to effect the H1 receptor on the cell, thereby preventing allergic attacks;
(2) Sedative hypnotic effect: the mechanism of inhibiting central nervous activity is still unclear;
(3) antitussive effect: It can directly act on the cough center of the medulla oblongata and inhibit the cough reflex.
pharmacokinetics
Oral absorption was complete, tmax was 2h, maintenance was 4-6h, elimination half-life was about 4h, protein binding rate was 78%-99%. 98% bind to plasma proteins. First pass metabolism is performed in the liver. Approximately 50% is metabolized before reaching systemic circulation after oral administration. It is widely distributed in brain tissue through the blood-cerebrospinal fluid barrier. Most of the intrahepatic transformation is excreted by urine, stool, sweat in the form of metabolites, and some of the lactating women can also be excreted by milk. Diphenhydramine can induce drug enzymes and accelerate its own metabolism. Almost all discharge within 24 hours.
